Rehabilitation and Visiting Nurse Association (RVNA) serves Weld and Larimer Counties in Colorado. Established in
1979, the agency prides itself on its history, and its excellent reputation as a quality, community based, home care service provider.
The mission of Rehabilitation and Visiting Nurse Association is to provide quality home health care and related services to the residents of northern Colorado with skill and compassion in a professional manner.
Our professionals provide individualized care to the patients and their families in homes as well as in various alternative housing settings such as assisted living and independent living communities. Quality services include: Nurses, Physical Therapists, Occupational Therapists, Speech Therapist, Medical Social Workers, Certified Nursing Assistants and Personal Care Providers.
It is our philosophy to work closely as a team with physicians, hospitals, hospices, other community agencies, and housing services in our area to ensure our patients receive the appropriate care necessary for them to achieve positive health outcomes. RVNA is a Medicare/Medicaid certified agency. It is state licensed and nationally accredited. The agency also provides services for private insurance and private pay patients.
Agency Information
Rehabilitation and Visiting Nurse Association is a voluntary, community based, not-for-profit corporation providing high quality home health services including, but not limited to nursing, physical therapy, occupational therapy, speech language pathology, social work, home health aide care and other community health services to the people of northern Colorado. The agency explores and develops health related programs based upon community need and acceptance, economic viability, and availability of resources to provide the services. The agency is sensitive to the financial circumstances of the patients, including services at reduced or no charge rates to indigent people within the constraints of available community support resources. Grants are received from Larimer County, City of Fort Collins, City of Loveland, Weld and Larimer Area Agency on Aging organizations, and Weld United Way.
History of RVNA
RVNA has been in existence since August 31, 1979, when the first patient was opened to services. Prior to that time, the only home health care services available in Weld County were provided through the Weld County Health
Department. A board of citizens whose function was advisory in nature gave community input into the scope of services and policies of that program. Final decisions were by the County Commissioners. Events of the two preceding years had led to a decrease in funding, fewer staff and a reduction in services. A feasibility study of health care providers, community leaders, and consumers determined such an agency would have community support. Thus, the formation of the Rehabilitation & Visiting Nurse Association. RVNA expanded into Larimer County in 1986 when the Larimer County VNA ceased operations and RVNA assumed responsibilities for their clients in the area.
RVNA Home Care Services, RVNA's sister organization, was started on March 14, 1986. This corporation was in response to the needs and requirements of the expanding RVNA. In order to continue to provide the maintenance-type services demanded by clients and still meet the Medicare requirements, it was apparent a new agency should be incorporated and devoted almost exclusively to the maintenance care of the elderly and handicapped.
With changes in Medicare requirements and payments, the two agencies merged under the auspices of RVNA Home Care Services on January 1, 2003. The combined agency was named Rehabilitation and Visiting Nurse Association.

Since 1979, Rehabilitation and Visiting Nurse Association has depended on community support to aid their mission of home health care to people of all ages. Donations allow us to be sensitive to patients ability to pay, and support current programs, as well as the development of new health programs, and help purchase supplies and equipment.
